Sports physicals may be obtained during a "Sports Phyisical Night" at River City High School, from your doctor, or may be available through one of the providers listed below. Please contact them directly for additional information or to make an appointment.

NOTE: All athletes must have a sports physical annually to participate in sports at River City.

Print and complete this form with appropriate physician, parent, and athlete signatures and submit it to your coach or the Athletic Director (Char Ghio). A yellow EMERGENCY FORM CARD is also required and can be picked up in the River City HS office.
